Private Fee For Service plans (PFFS) are Medicare based plans that allow flexibility to see any provider in or out of network. The provider must be presented with your plan information before being treated. The providers may see you if they accept the plans terms and conditions. If the provider does accept the plan, then they must not charge more than the cost sharing amount in your plans Evidence of Coverage. The copayments must not exceed the terms offered by the plan.
