Medicare Special Needs plans are available in counties across the Carolinas. There are two types of special needs plans. One may find the benefits of a special needs plan that caters to those that may have chronic conditions such as diabetes or heart failure. These are referred to as C-SNP’s. They often can provide additional benefits that can help manage chronic conditions and keep members healthy. Another type of special needs plan are Dual Eligible plans, known as D-SNP’s. These plans are designed for the population that has Medicare and also qualifies for Medicaid cost-sharing. These plans benefit members by offering services and benefits that Medicaid and Medicare may not cover. Finally, there I-SNP’s, which are designed for people who have Medicare and that are in an institutionalized setting such as a nursing home.
